Note that the First Name and Last Name labels occur on the same line, but
that the text boxes are on the line above. Similarly, the radio buttons are
placed below the name of the location for a meeting. The screen reading
technology generally assumes that the text immediately to the left of an input
element is the label for that element. Thus screen reading technology would not
be of much assistance to a visually impaired individual filling out this form.
